Pterygium is a disorder that occurs by an overgrowth of the proximal nail fold on to the nail bed. In severe cases the nail plate is eventually replaced with scar-like tissue. The condition typically effects the nail matrix, nail bed and nail plate but sometimes all parts of the nail unit are involved. Two types of the condition can occur. Deformities are often considered together with dystrophies, but the two are slightly different; deformities are generally considered to be gross changes in nail shape, whereas dystrophies are changes in nail texture or composition (eg, onychomycosis ). About 50% of nail dystrophies result from fungal infection. Pterygium is an irreversible scarring of the nail matrix resulting in a split nail. The deformity is most often post-traumatic, though it can be the end result of aggressive inflammatory diseases involving the nail matrix like nail lichen planus, graft versus host disease, or autoimmune bullous disorders like cicatricial pemphigoid. Medical literature defines pterygium (terr-idge-ee-uhm) as any abnormal growth of skin that becomes stretched into a wing-like shape.
