Damoiseau-Ellis line is a clinical sign, that refers to upper limit of dullness to percussion caused by pleural effusion. It has characteristic shape of curved line with highest point at middle axillary line 1. The eponym Damoiseau-Ellis line is sometimes erroneously used as description of pleural effusion apperance on erect chest x-ray. According to one prospective observational study, the most common diagnoses among elderly patients presenting to an ED with a complaint of acute shortness of breath or dyspnoea are decompensated heart failure, pneumonia, chronic obstructive pulmonary disease, pulmonary embolism, and asthma ( 1 ).

Classification Light's criteria are used to categorize effusions and to guide further workup. An effusion is classified as exudative if any of these 3 criteria are met: oleothorax pseudochylothorax urinothorax (urothorax) Epidemiology As the accumulation of fluid in the pleural space occurs in a broad range of disparate clinical scenarios, no single demographic is affected; rather the epidemiology will match that of the underlying condition. First Online: 01 February 2020 1322 Accesses Abstract A chest radiograph—or chest X-ray (CXR)—is the most commonly requested first-line examination for the diagnosis of numerous clinical scenarios thanks to the widespread availability, low radiation doses, and modest costs.

It is mobile, non-ionized, and non-invasive real-time exploration. Thoracic ultrasounds potentially augment the physical examination of the chest and represent a supplement to conventional techniques.

The upper border of simple pleural effusions has been described by many prominent clinicians during the past century, particularly Wintrich, Skoda and Niemeyer. Of all descriptions, the one familiar to most is the curved line of flatness of Damoiseau 1 and the line of Ellis 2, an excellent account of which can be found in Garland's monograph. 3. Larger quantities of fluid present with a Damoiseau-Ellis line (Fig. 10.2b), a contralateral shift of the mediastinal structures and the accumulation of fluid within fissures in the LL view. In a supine patient the fluid collects mostly in the posterior regions in a gravity-dependent pattern. Therefore, it may be difficult to observe on chest X.
